Reporting Forms
Required NC Campaign Reporting Forms
In February 2002, new campaign reporting forms were developed to make it easier to disclose and understand the various types of required information. On this page you will find all the new forms, divided into their different categories as set by the Campaign Reporting Office.
- Statements of Organizations for Registered Political Committees - various forms that do not fit a specific category, such as the Statement of Organization
- Registered Political Committee Disclosure Reporting Forms - the forms that can comprise a NC campaign reporting disclosure report
- Certifications - certifications that a treasurer or candidate may have to file
- North Carolina Public Funding Program Forms - forms required to qualify
- Federal Forms - forms federal PACs are required to file
- Notices - complaint notices
- New Candidate, Party or PAC Forms - forms required by new candidates or committees
The Contributions from Individuals Form CRO-1210 requires occupation and employment information. This following link includes the specific field codes (taken from the NAICS). These are one option to use when filing out this portion of the form.
